# GLP-1 explained in simple terms

GLP-1 stands for **glucagon-like peptide-1**, a hormone your body produces naturally in the gut after you eat. It plays a central role in how your body manages blood sugar and hunger. Medications that mimic or enhance this hormone have become one of the most clinically significant advances in metabolic health treatment in recent years.

## What GLP-1 does in your body

When you eat, your gut releases GLP-1 as part of a normal digestive response. This hormone does three things that matter most for blood sugar and weight:

- **Increases insulin secretion in a glucose-dependent manner.** The hormone prompts your pancreas to release insulin only when blood sugar is elevated, which reduces the risk of blood sugar dropping too low.
- **Slows gastric emptying.** Food moves more slowly from your stomach into your intestines, which flattens the spike in blood sugar after a meal.
- **Reduces appetite through central satiety signals.** GLP-1 acts on receptors in the brain that regulate hunger, helping you feel full sooner and stay satisfied longer.

Together, these effects make GLP-1 relevant for both type 2 diabetes management and weight loss.

## How GLP-1 medications work

GLP-1 receptor agonists are medications designed to activate the same receptors that your natural GLP-1 hormone targets, but with a longer duration of action than the hormone itself. Natural GLP-1 breaks down within minutes. Medications like semaglutide are engineered to last much longer, which is why they can be taken once weekly rather than continuously.

The result is a sustained reduction in appetite, more stable blood sugar levels after meals, and, over time, meaningful weight loss for many people. For a closer look at how semaglutide specifically works, see [What is semaglutide?](/sugarfit-what-is-semaglutide).

## GLP-1 versus dual-agonist medications

Some newer medications, like tirzepatide, act on two hormone pathways instead of one, making them a next-generation incretin therapy. Clinical studies have shown greater average weight loss and blood sugar reduction with tirzepatide compared to GLP-1-only medications, which is a meaningful distinction for people who have not achieved their goals on a GLP-1 alone.

| Feature | GLP-1 receptor agonist (e.g. semaglutide) | Dual agonist (e.g. tirzepatide) |
| --- | --- | --- |
| Receptors targeted | GLP-1 only | GLP-1 and GIP |
| Insulin sensitivity effect | Moderate | Enhanced via GIP combination |
| Average weight loss | Clinically significant | Greater in head-to-head studies |
| Classification | GLP-1 therapy | Next-generation incretin therapy |

## Who GLP-1 therapy is typically for

GLP-1 therapy is generally considered for adults who have not achieved adequate results through lifestyle changes alone. The clinical reason for moving to medication is the risk of metabolic complications when weight and blood sugar remain uncontrolled over time.

In an Indian population context, eligibility thresholds are lower than global averages because health risks may begin in the 23 to 25 BMI range for South Asians. ICMR recognizes this population-specific risk profile. Typical eligibility criteria include:

- BMI of **30 or above**, or
- BMI of **25 or above** (applies to Indians) with at least one weight-related condition such as type 2 diabetes, high blood pressure, or dyslipidaemia

A 3 to 6 month trial of structured lifestyle intervention is a clinically recognized standard before medication is introduced, unless metabolic risk is high enough to warrant earlier treatment. If you want to assess whether this applies to your situation, [Is GLP-1 right for you?](/sugarfit-is-glp-1-right-for-you) covers the eligibility question in more detail.

## What to expect from treatment

GLP-1 therapy requires dose escalation, ongoing monitoring, and lifestyle support. It is not a standalone solution. Most people start on a low dose that is gradually increased over several weeks to reduce side effects and allow the body to adjust.

Early changes in appetite and blood sugar response are often noticeable within 4 to 8 weeks. More significant outcomes, including measurable weight loss and improved metabolic markers, typically become clear over a 3 to 6 month window.

**Side effects** such as nausea, reduced appetite, and digestive discomfort are common, particularly in the initial weeks or after dose increases. These side effects tend to improve over time and are usually manageable with gradual dose adjustment. If they persist or are severe, a doctor should be consulted.

GLP-1 therapy is not appropriate for everyone. People with a personal or family history of medullary thyroid cancer, a history of pancreatitis, or severe gastrointestinal disease should discuss these conditions with a doctor before starting treatment, as these are recognized contraindications.

## Lifestyle changes versus medication

Dietary changes, increased physical activity, and sleep improvement can support weight loss and blood sugar control, but their effect is meaningfully smaller in magnitude compared to GLP-1 medications for people with significant metabolic risk. Lifestyle changes remain an important part of any treatment plan, but they are unlikely to replicate the clinical outcomes seen with supervised GLP-1 therapy in people who meet the eligibility criteria above.

## Getting started in India

Supervised GLP-1 programs combine doctor consultations, dose management, and ongoing monitoring. Early side effects like nausea, reduced appetite, and digestive discomfort are usually temporary and improve with gradual dose adjustment.
